In The Evil Within “The Patient,” I introduce my strategy for finding the location of 🗺️Map Fragment 5 at Elk River Hospice, as well as fragments 6 and 7 in the hospital, 📙the Villager’s Note, 🔥saving a soul caught in front of a bonfire, 🩻 key locations, and more.

In this guide, you can acquire

🗺️Elk River Hospice Area Town Map
While talking with the doctor, you are guided to move to House 2 at position 4️⃣, but for the best route, it’s recommended to follow the order of locations below. Upon first arriving and discussing where you are with the doctor, there are loot boxes on both the left and right from where you stop.
1️⃣Hut 1, 2️⃣House 1, 3️⃣Bonfire
4️⃣House 2, 5️⃣Hut 2, 6️⃣House 3
In the game scenario, the indicator showing House 2 as the destination can be distracting, so just press the spacebar to acknowledge it.
🗺️Map Fragment 5+
Head to position 1️⃣ on the map above, where you’ll find a hut at the top of the stairs. As you approach the stairs, a sturdy monster will break through the door with an axe. I recommend shooting it once in the head with a shotgun, and when it falls, quickly rush over and burn it.
Inside the place where the monster was, you’ll find Map Fragment 5, a flash bolt, a bomb box, and revolver ammunition.
📙Villager’s Note
Now, if you go to House 1 at position 2️⃣, as you approach the door, a screaming burning female ghost will emerge and disappear, leaving behind green gel.
Once inside the house, you’ll find matches at position 1 on the map, an loot box and a gap at position 2, revolver ammunition at position 3, a green gel bottle and “📙 Villager’s Note” on the bed at the farthest point at position 4.
As shown in the bottom left screenshot, you can approach the gap and press the spacebar to enter. The note is on the bed, as shown in the bottom right screenshot.
Behind the picture on the wall, you’ll find a syringe.
🔥Saving Poor Souls
At position 2 on the map below, a monster throws the spirit of a woman onto a bonfire. If you save her, you can collect 1,000 green gel.
You can jump over the fence in front of Hut 2.
Grab the axe on the table in front of Hut 2, then head to position 2 on the map above. As circled in the bottom right screenshot, you’ll see a monster carrying the spirit of a woman towards a bonfire.
As soon as you see this happening, sprint briefly and hit the monster with the axe, as shown below, for a clean rescue.
The saved soul will flee to Hut 3 at position 3 on the map above. Follow her, and as shown below, she will turn into green gel after kneeling. Beside her, you’ll also find 2 harpoon bolts.
Now, head to position 4 on the map, where there are bodies hanging and some green gel on the ground. You don’t need to burn the bodies.
⚔️Dr. Valerio Jimenez
At position 4 on the map, as you make your way there, you’ll find a syringe, a green gel bottle, and an loot box nearby.
When you reach position 4 on the map, after a cutscene, Dr. Valerio Jimenez will attack. Since he attacks up close, shooting him with a shotgun and then quickly burning him is the easiest method.
Alternatively, you can use flash bolts found throughout the game to attack. Flash bolts don’t need to hit directly; they temporarily blind all nearby enemies, allowing you to aim and shoot immediately.
When the doctor is dazed, as shown on the bottom left, crouch and perform a “sneak kill” while he’s blinded. This method works on most humanoid monsters. If the flash bolt’s duration is long enough, you can use this approach on up to 3 enemies in the vicinity.
For large monsters like the one shown below, even a shotgun headshot may require two shots. But with this method, you can take them down using just one flash bolt and a stealth kill, conserving ammunition.
🩻Key Locations
There is an X-ray image next to the operating table. Upon examination, it shows the location of a key inside the corpse.
As indicated on the bottom right, aim the scalpel, and when the hand icon appears, press the spacebar. After a gruesome and startling cutscene, you will acquire a key.
As shown on the map, there are matches at position 1, and a green gel bottle at position 2, as indicated in the bottom right screenshot.
When you go up to the second floor, directly ahead in the room reachable via the stairs, you’ll find a flash bolt, a green gel bottle, an loot box, and revolver ammunition on the left shelf, as seen below.
Two connected rooms are visible, and entering through the left door, you’ll encounter a monster facing away but that soon turns around. I recommend using the flash bolt + sneak kill technique to take it down. Nearby, as soon as you enter the left door, there is a green gel bottle on the desk straight ahead, and shotgun ammunition on the shelf in the middle of the room.
From there, head down the left corridor, and you’ll find matches on a sofa near the exit, as shown below. After collecting the matches, exit through the nearby door.
📒Sebastian’s Journal: February 2005
After heading down, you’ll find yourself in front of a hut with a mirror that leads to the hospital.
“📒Sebastian’s Journal: February 2005” is pinned to the door of the hut.
💕Hospital
During your first visit to the hospital in this chapter, the screen will appear in black and white with loots visible as shown below. During subsequent visits, Leslie’s room will be locked, and the rest will return to normal.

In the lobby where the nurse is, you’ll find the 📰 newspaper “Missing Patient Found” and 📜 Missing “Valerio” at positions 1 and 2.

In Sebastian’s room, you can find “🗺️Map Fragment 6” at position 3 on the map above.
🫣 Oh! Nurse!
Now, at the location of mirror number 4, the nurse is standing with her back turned. The bottom left shows the paid Korean patch version, and the right shows the English version. In the Korean-patched version, the issue with the nurse’s skirt riding up is fixed, but in versions without the Korean patch or with unofficial Korean patches, it appears as shown on the right.
Bethesda hasn’t changed; they only update things if they get paid!
🗺️Map Fragments 7
While following the nurse, Leslie’s room will open. When you enter and reach the sink, the atmosphere will change, and the room will appear about three times longer. At this moment, “🗺️Map Fragment 7” will appear above the sink, as shown in the bottom right screenshot.
Map Fragments 6 and 7 can only be obtained at this point in the game.
When you reach Leslie, an event will surprise Sebastian, and the hospital will return to a normal state. To aid in your escape from Laura in the upcoming segment, it’s recommended to upgrade “Maximum Sprinting Time” to level 3 or higher.
